Abstract
Despite their greater burden of disease, older adults are largely underrepresented in randomized clinical trials evaluating percutaneous coronary intervention, a minimally invasive procedure that improves myocardial perfusion and anginal symptoms. This is particularly important given that (1) chronic total occlusions affect more than one-fifth of patients who undergo diagnostic angiography and (2) older adults have the largest burden of chronic coronary disease leading to significant disability, morbidity, and mortality. We propose to study the effects of percutaneous coronary intervention for chronic total occlusions in adults 75 and older on quality of life compared with optimal medical therapy. Patients at 50 centers in the United States will be randomized into either the intervention or control group and followed for one year. This information will provide evidence to support the preferred therapeutic approach in older adults with chronic coronary disease and chronic total occlusions.
